Congregate Living Health Facilities (CLHF)
MISSION STATEMENT
The DreamCatcher Foundation, Inc provides financial support to Congregate Living Health Facilities(CLHF) who provide a continuum of care which includes, but is not limited to sub-acute care, skilled nursing care, and long term care services to the underserved and underinsured patients being discharged from hospitals and other medical facilities.
SERVICES
Congregate Living Health Facilities (CLHF) are “residential” and provide a home-like environment, usually six or twelve beds. CLHFs provides 24 hour medical supervision by a comprehensive team of medical professionals to include a Medical Director, RN, LVNs, CNAs, Respiratory Therapists along with Physical, Occupational, and Speech Therapists, to name a few.
CLHFs are fulfilling three critical needs as listed below:
Medical - Ventilator dependent patients, wound care management, diabetic management, intense physical rehabilitation, tracheostomy care, gastrostomy tube care, etc....
Housing- Social workers to assist with obtaining long term housing placement
Nutrition- Full course meals regulated by dietitians and the Department of Health and Safety.
