Resources for underserved underinsured patients

Patients exiting private hospitals and medical facilities who are underserved and or underinsured are in need of community congregate care facilities that can meet their medical needs. The Dream Catcher Foundation, with the help of donors, can locate and provide financial assistance in placing patients in a CLHF in the community.
